Recruitment for Community Health Centers to participate in the Health Resources and Services Administration (HRSA) for the fiscal year (FY) 2025 Health Center Controlled Network (HCCN) Cooperative Agreement will continue throughout the funding cycle.

The purpose of this funding is for HCCNs to support health centers in leveraging health information technology (IT) and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care, with new goals areas consisting of:

- Interoperability and Data Sharing
- UDS+ Implementation
- Value-based Care
- Digital Health Tools
- Strengthening Cybersecurity Support
- Artificial Intelligence
- Social Risk Factors
The funding will cover the period of August 1, 2025 - July 31, 2028.
