Duplicate medical records can greatly impact end users struggling to identify the correct chart to use while documenting within a patient chart. These examples include but are not limited to compromising patient safety (ie: medications/allergies), creating unnecessary gaps in care, discrepancies in medical information available to the patient on their own patient portal, inaccurate patient statements/balances, challenges in detailed population health management review/ data analysis, and reimbursement.

Since Medication Assisted Treatment (MAT) may be provided by a variety of provider types, clearly outlining these within the electronic health record (EHR) is essential for accurate data capture and reporting. This article can guide practices in the configuration of providers within their electronic health records to ensure reporting requirements for MAT services are captured and reported accurately. Though UDS is the primary program discussed, the information provided within this article will be beneficial for various other quality programs where MAT services must be reported.
